Fill out the application form and enclose two photos. Present a valid ID or passport. 2. Pay the membership fee: NT$1,000 for six-month members, or NT $ 1,800 for one-year members. 3. To apply membership by mail: send 2 photos, one photocopy of valid passport or resident ID, a postal money order of appropriate amount, one self-addressed stamped envelop with your contact address and phone number to 4F, No. 7 Ching-Tao East Road, Taipei, Chinese Taipei Film Archive. Please indicate this is an application of membership. YOUR PRIVILEGES AS A MEMBER 1. Free attendance of the monthly programs, special 20% discount of the books published by CTFA. 2. For six-month members and one-year members: free use of the collection of the archive, including periodicals, newspapers, viewing videotapes in the screening booths, and checking out books. 3. Free subscription of the quarterly, Film Appreciation Journal: 4 issues for one-year members, 2 issues for six-month members, 1 issue for seasonal members. NOTE 1. The membership card is not transferable. Any change to the membership card will not be accepted. Show the valid membership card when you enter the archive. 2. Drinking and eating is not allowed. Do not come in and out the auditorium during the screening. If the screening has begun for 30 minutes, please do not enter to the auditorium. 3. The members can loan two books each time. The reference books are not for loan. To loan the foreign language books, a deposit NT$ 1,000 for each is required. The deposit will be given back immediately after the books are returned. The books are due in three weeks. For each expired day, each book will be charged NT$ 10. The overdue notices will be mailed to notify the return of the expired books. If books are not returned after three overdue notices, the membership will be canceled. Only after the books are returned and the expired charges are paid, one may apply for membership again. (holidays and Sundays are included in the overdue dates¡FIf one¡¦s membership will be expired in three weeks, the member can not loan books, or one has to give a deposit for each book) 4. If the loaned books are lost or damaged, the borrower shall purchase a copy of the book to replace the lost or damaged copy, or shall compensate by paying an amount equal to twice the original purchase price. In the case of books no longer in print, compensation shall be in the amount of four times the purchase price. 5. To loan and view the video (tapes, LDs, VCDs, or DVDs ) fill out the request slips and give it to the staff. The staff will retrieve the video. Only one video may be loaned at a time. 6. Notices for viewing the videos. „X Keep quiet during viewing „X If leave during or after the viewing, please turn off the power and return the headset to the counter. 7. If members lose membership cards, one has to pay NT$100 for a new card. OTHER NOTICES: 1. The membership card is not transferable and once issued, it¡¦s not possible to ask for refund. 2. Please follow the REGULATIONS, any violations or making-noise in reading/AV room, the membership will be suspended for 1 month. If violated again, the membership will be withdrawn and the person could not apply for membership during one year.
